Friends of Teen Arts, Inc. Artwork and Writing Release Form

            In consideration of the acceptance by the Friends of Teen Arts, Inc., the New Jersey State Teen Arts Festival, the New Jersey Teen Arts Touring Art Exhibit, and/or any other program sponsored by the Friends of Teen Arts, Inc. (collectively “Teen Arts”) of my art piece or writing piece, for display and/or publication, and intending to be legally bound, I hereby grant Teen Arts my consent for the public and private use of my art or writing piece in accordance with its rules and policies.

             I hereby release Teen Arts, its officers, agents, funding organizations, grantors, and public and private collaborators from any and all liability for damage, loss, or misappropriation of my art or writing piece.  I further represent that this art or writing piece is an original work of art (originally conceived and executed, not copied from any other artist’s or photographer’s image) or writing personally executed by me, for which I have secured, or am entitled to secure, a copyright.

             I further grant Teen Arts, its officers, agents, funding organizations, and grantors, and its private and public collaborators, the right to include this work in a derivative work, such as a compilation on which this work is depicted as one representing Teen Arts, in a book, pamphlet, calendar, anthology, or any other literature form for the benefit of Teen Arts.

             Notwithstanding the above, I retain all proprietary rights, including but limited to, legal title and beneficial copyright ownership in my art or writing piece during and subsequent to the time that it is in the possession of Teen Arts. I hereby grant Teen Arts, its officers, and agents a gratuitous license to use the art or writing piece and to photocopy or otherwise reproduce it. I agree to indemnify, hold harmless and defend Teen Arts, its agents, officers, funding organizations, grantors and public and private collaborators against any and all claims of any nature whatsoever in connection with the display and/or publication of my art or writing piece, including any relating to copyright infringement, by any party whatsoever, arising out of or in any way relating to this agreement and the display of my art or writing piece.  I agree that the final decision regarding the suitability of my art or writing piece to be used will be made by Teen Arts.

            I agree to allow my art or writing piece to be used by Teen Arts according to the terms herein and its rules and policies from June 1, 2008 through September 30, 2008.
